This paperwork must be completed for students who have been accepted in the 5-Day 4s Class for the 2025-2026 school year. Please note the last section of the paperwork re: committees & family helper days is not required for students in this class.

Please ensure this paperwork is submitted and that your first installment of tuition has been paid by Thursday, May 15.
